Abstract

Medication error that frequently harms the patients can be preventable. It can lead
to inappropriate drug use in which actual drug use should be under supervision of
health personnel. The objective of research is to determine the percentage of
medication error in the process of prescribing, transcribing, and dispensing the
compound prescriptions in community health center of West Region Banyumas.
This research is a descriptive research with cross-sectional approach. The sample
of research was 100 sheets of compound prescriptions with prospective data
collecting technique. The research was conducted from February-April 2018. In
prescribing process, there were errors: no dosage information (98%), no dosage
strength (96%), no patient’s weight info (95%), no prescriber’s name(72%), no
dosage form (65%), no rules of drug use (64%), no method of making dosage
(42%), no prescriber’s initial (17%), no patient’s address info (17%), no patient’s
age info (10%), no amount of drug (4%), and no prescription date (1%). In
transcribing process, there is patient’s name error (1%). In dispensing process,
there were errors: no contradiction (100%), no beyond use date (90%), no properstore
drug info (85%), no side effect inclusion (80%), no labels (48%) and no
compatible drugs (9%).
