The Covid-19 pandemic that is currently spreading around the world has had a huge impact on all aspects of life, including education. The Covid-19 pandemic has an impact on students' ability to be disciplined, such as in collecting school assignments, doing homework, and so on. The traditional face-to-face learning process has transformed into remote learning. There are many different models and mediums for remote learning. One of the media used by SD Negeri 2 Cilongok for the learning process is the Google Form application. This study aims to describe the use of Google Form application to improve students' discipline for the fifth grade of SD Negeri 2 Cilongok during the Covid-19 pandemic and to find out the supporting and inhibiting factors of the use of the Google Forms application. Observation, interviews, and documentation were used to collect data. Students of SD Negeri 2 Cilongok served as the sample of the study. Although the researcher is the major source, the development of supporting instruments is not ruled out. Triangulation techniques and sources were used to check the data's validity. The findings show that using the Google Form application in the classroom during the Covid-19 pandemic can help teachers in assessing student discipline in collecting assignments and class attendance because the system is very easy to use and can motivate children to complete and collect school assignments. The use of the Google Form application is affected by supporting and inhibiting factors, including 1) the use of science and technology 2) the use of learning media 3) in accessing the internet network.
