Abstract

each year. Based on Riset Kesehatan Dasar data in 2013, the number of CHF sufferers in Central Java province ranks second most after West Java province. DRPs are defined as an event induced by the treatment of drugs either significantly or potentially interfering with the desired therapeutic result. This study aims to determine the problem of treatment associated with the incidence of DRPs in CHF patients in hospitalized hospitals Prof. Dr. Margono Soekardjo Purwokerto. Sampling using descriptive-observational method with prospective approach. The identification of DRPs is done in accordance with Pharmaceutical Care Network Europe (PCNE) V.07. The result of the research shows that the problem of treatment effect is not optimal (P1.2) 38,87%, unnecessary medication and treatment (P1.3) 16,08%, untreated indication (P1.4) 2,95%, And the presence of adverse drug reactions occurred (P2.1) 42.09%. The cause of DRPs was highest due to drug interaction (C8.2) 52%, inappropriate medication according to guidance / formulary (C1.1) 11.71%, drug dose too low (C3.1) 8.70%, inappropriate drug or contraindicated (C1.2) 6.86%, no indications for drug (C1.3) 5.18%, indication for unregarded treatment (C1.6) 3.51%, The dosage of the drug is too high (C3.2) 4.52%, the frequency of the dose regimen is low (C3.3) 4.01%.
