Abstract

Background: The use of antibiotics in COVID-19 patients with pneumonia is still not rational. This is due to several factors, including inappropriate dosing resulting from increased doses during prolonged intensive care, the failure to perform bacterial culture tests, which leads to the use of antibiotics that are not specific for eradicating the bacteria in question, and an increased risk of antibiotic resistance. This study aims to determine the pattern and rationality of antibiotic use in COVID-19 patients with pneumonia at RSD Gunung Jati Cirebon. Methods: This research uses a non-experimental design of medical record data. Data was obtained retrospectively in RSD Gunung Jati Cirebon period from January 2020 – October 2023. The number of samples used is 97. The inclusion criteria for this study are patients who are treated intensively in the ICU, receive antibiotic treatment during their care, and have chest X-ray results and Polymerase Chain Reaction (PCR) tests. Meanwhile, the exclusion criteria are patients who leave at their request, die, or are referred to another hospital. Data analysis is carried out univariately to obtain an overview of the characteristics of the patient and concluded in the form of a percentage. The antibiotic use evaluation method uses the Gyssens flow diagram method. Results: Out of 97 medical records, data indicated that the highest prevalence was in male patients (55%), age >45 (24.7%) with coagulopathy (21%), and DM 2 (21%). The most commonly used type of antibiotic was azithromycin (74%). The results of the evaluation of the Gyssens method were obtained in category 0 (100%). Conclusion: The use of antibiotics in Covid-19 patients with pneumonia at RSD ICU Gunung Jati Cirebon is rational.
