The Jetis Health Center, which still has limited adequate facilities to provide health services for patients, really needs a better information system. This is evidenced by the use of a conventional system where medical record data is recorded in a book. The use of book media causes several obstacles such as requiring a place to store books, the data search process takes a long time and is also vulnerable to damage, be it torn or exposed to water. The purpose of this study was to design and build an information system for the Jetis Health Center, Madiun Regency. The system development method used in this study is the waterfall method. The waterfall method takes a sequential or sequential software lifeflow approach where the model generally starts from analysis, design, coding, and testing. The result of this research is the puskesmas information system. The conclusion in this study is that the Jetis health center information system in Madiun district makes it easier to process data and generate reports. Data search easier and faster. Make it easier to access the system because the system is website-based so that it can be accessed online. Make it easier for patients to register for examinations at the puskesmas.
