Abstract

Lung disease is a disease that is still a serious problem in health in
the world and in Indonesia. The type of concoction prescription is more dominant
in giving to pulmonary poly patients. Prescription concoctions have the potential to
cause medication errors. Medication errors in compounded prescriptions often
occur in the prescribing phase. Objective: To determine the percentage of
Medication Error incidence in compounded prescriptions at Kardinah Tegal
Hospital. Methods: This analysis is a quantitative analysis that aims to understand
the incidence of medication errors on the prescribing phase, dispensing phase and
administration errors contained in the prescription of concoctions at Kardinah
Hospital Tegal. Results: Medication errors in the prescribing phase, namely there
is no doctor's SIP (100%), no weight (BB) patient (100%), and there is no patient's
height (TB) (100%). In the dispensing phase, no medication errors occurred.
Medication errors in the administration phase were not explaining how to use
(27,09%), not explaining the time of drug administration (71,57%), patient name
errors (1,00%), not explaining drug indications (100%), not explaining
contraindications (100%), not explaining side effects (100%), not explaining
Beyond Use Date (100%), and not explaining drug storage (100%). Conclusion: in
the prescribing phase showed 33% incidence, in the dispensing phase 0% and in the
administration phase 67%.
