Abstract

Background : Medication reconciliation is a pharmaceutical cares and in there is
the process of comparing the obtained patient treatment instructions. Medication
reconciliation plays on important role in reducing the incidence of medication
errors in particular drug interactions and potentially dangerous to the patients. A
study in Nepal known drug-drug interaction events by 53% in patient in internal
medicine wards and ICU with and average of 8,53 of drug users by prescription
medication. In the another study also showed that medication reconciliation
proven to be a effective strategy to reduce the incidence of drug interactions.
Therefore, the presence of medication reconciliation can reducing the incidence of
drug interactions and can improving patient treatment outcomes.
Research purposes : Measure the influence of medication reconciliation to the
reduction of potential drug interactions in hospitalzed patient on RSUD Prof Dr.
Margono Soekarjo Purwokerto.
Research methods : This research was a descriptive study with prospective data
collection methods through observation based on patient medication reconciliation
sheets in RSUD Prof. Dr. Margono Soekarjo Purwokerto.
Result : Samples used in this study were 95 patient in RSUD Prof. Dr. Margono
Soekarjo Purwokerto. From 95 patient, there were 52 patient (54,73%) with the
drug interaction potentials, after medication reconciliation process there been a
decrease of 20 patient (21,05%) with no drug interaction potential in their
treatments, while 32 patient (33,68%) still have potential drug interaction in their
treatments. And based on the decision of continuing in medication reconciliation,
there were 43 cases drug interaction with the severity of minor 72,10% (31 cases)
and the severity of moderate 27,90% (12 cases). And 15 type of drug interactions
with pharmacodynamic interaction mechanism 80% (12 type of drug
combinations) and pharmacokinetic interaction mechanism 20% (3 type of drug
combinations).
Conclusion : The incident of drug interactions that exist are 52 patients (54,73%)
after medication reconciliation processed there were decrease of 20 patients
(21,05%) with no drugs interaction, while 32 patients (33,68%) still has drugs
interaction. Thereforepharmaceutical cares as a medication reconciliation in
RSUD Prof. Dr. Margono Soekarjo Purwokerto in not optimally, because there is
still a drug interaction in patient prescriptions after process of mediaction
reconciliation occurred.
