Cervical cancer is a deadly disease in the World as well as in Indonesia. Cervical cancer is mainly cause by Human Papilloma Virus type 16 and 18. To prevent of occurrence cervical cancer, one of them doing primary prevention by use HPV vaccine. HPV vaccination can the protect by body 89%. HPV vaccination will be included national immunization. The aims of this study was to know of knowledge, attitude about HPV, cervical cancer and HPV vaccination, to know acceptance HPV vaccination to health care provider in Purwokerto, to know relation of knowledge and acceptance, to know relation of attitude and acceptance. The method of this study was cross sectional analytical survey with accidental sampling with 54 respondents. The result of this study knowledge HPV and cervical cancer (68%), attitude to HPV vaccination (76,05%), and acceptance of HPV vaccination (83%). The difference respondents of knowledge with occupation p-value 0,031 (p<0,05). Correlation of knowledge with acceptance p-value 0,02 (p<0,05). Correlation of attitude with acceptance p-value 0,338 (p>0,05). Umur is the most dominant variable related to acceptance HPV vaccine with OR value of 1218
