Abstract

Medication error is a harmful-error that occurs when patients is handled by professional health personnel due to inappropriate use of drugs which can be prevented (NCCMERP, 2018). According to WHO in 2016, medication error can occurs in various health services, especially primary care. The study of medication error in primary care in community health centers is rarely found (Dwiprahasto, 2016). This study was conducted to find out the percentage of medication error occurrence in the process of prescribing, transcribing and dispensing compounded medicine in community health center in East Region of Banyumas. This study was categorized into quantitative descriptive research done by observation using cross sectional approach. The sample of this study was 100 extemporaneous compounding recipe obtained prospectively by using purposive sampling. The observation was performed at the stages of prescribing, transcribing and dispensing using a checklist as a research tool. This study was conducted from November 2017 to January 2018. The results showed that the medication error occurred at prescribing stage (no name of prescription writer 64%, no signature of prescription writer 68%, no prescription date 1%, wrong/unclear patient name 2%, no / incomplete patient’s date of birth (age) 14%, no/ unclear patient’s address 25%, no patient weight 96%, no/ incomplete drug effect 99%, no / incomplete drug form 92%, no / wrong drug amount 3%, no / wrong drug usage rule 73%, incomplete / no / wrong way of making drug form 72%). In addition, medication error occurred at the transcribing stage (wrong patient’s name 2%, wrong patient’s age 1%, wrong drug name 0%, wrong drug form 0%, wrong dosage strength 0%, wrong drug amount 0%, inability to know drug dosage suitability 1%, wrong way of making 0%, incorrect use 0%).At the dispensing stage, medication error also happened (taking expired drug 0%, incorrect / unsuitable in making / taking form of drug 8%, wrong drug form strength 6%, wrong solvent type 0%, wrong drug amount 8%, incompatible drugs 0%, wrong drug 3%,incorrect / incomplete labeling and eticket 40%,unclear drug use instructions 10%, not delivering drug side effects 96%, not conveying drug contraindications 100%, not delivering beyond use date 86%, not conveying drug storage 92%).These results indicates that errors is occurs in the process of prescribing, transcribing and dispensing that can lead into medication error in prescription recipe at east Banyumas Community Health Centers.
